Consent & Waiver
I am the parent/legal guardian. I understand the Angry Birds Trampoline involves physical activity and risk of injury. My child is medically fit. I accept all risks and waive claims against Angry Birds World, its management and staff, except in cases of gross negligence. I agree my child will follow safety rules and I authorize first aid or emergency medical treatment if required.
